Sports injuries that occur to a person will cause a sensation of pain caused by damage to the structure or function of the body, such as bones, joints, ligaments and muscles. One type of sports injury that often occurs is rupture of the anterior cruciate ligament (ACL). The ACL ligament is an important component in the knee. The main function of the ACL is to maintain stability when anterior tibial movement and internal rotation occur. Rupture is a condition when tissue is damaged or torn due to trauma. Case Presentation: The patient experienced limited movement and pain when flexing and extending the left knee accompanied by pain which made it difficult for the patient to carry out daily activities. The patient suffered an injury caused by an opponent's attack during a silat competition which resulted in a “pop” sound which made his knee ache and made it difficult to bend his knee. The patient was medically diagnosed with left knee ACL rupture. The patient complained of motion pain and tenderness in the left knee area, flexor and extensor muscle weakness, hamstring muscle spasm, decreased joint range of motion, and muscle atrophy. Management and Outcomes: Patients are given intervention in the form of phase I exercise including: quadriceps set, hamstring sets, clump sell, SLR, ankle theraband, slide heels, weight training. Before the exercise, the patient is given tens and given an ice pack after the therapy. After being given the intervention once, the results showed that there was no significant decrease in motion pain, there was no significant increase in muscle strength, there was an increase in the range of motion of the joints, there was no increase in muscle mass. Discussion: Tens aims to help reduce pain, stimulate muscle activity, and reduce swelling, phase 1 exercise therapy, namely strengthening exercises quadriceps set performed to increase strength in the quadriceps muscles, hamstring sets done to increase strength in the hamstring muscles, clump sell performed to increase strength in the gluteus muscles and stabilization of the hips, SLR to strengthen the abductor, adductor, gluteus, and hamstring muscles, ankle theraband to increase the strength of the ankle muscles (m. tibialis anterior, m. gastrocnemius, m. peroneus), slide heels performed to increase knee joint ROM and stretch leg muscles, weight training to add LGS to the knee extension movement and to reduce pain, and ice packs to reduce pain and swelling, so that it can help increase range of motion (ROM).
2023-09-22T00:00:00+07:00 Copyright (c) 2023 https://proceedings.ums.ac.id/index.php/apc/article/view/3137 Physiotherapy Management of Medial Collateral Ligament (MCL) Repair: A Case Study 2023-10-14T13:19:07+07:00 Adetya Priyo Dewanto ram630@ums.ac.id Galeh Purwatama ram630@ums.ac.id Dimas Zena ram630@ums.ac.id Rinna Ainul Maghfiroh ram630@ums.ac.id
Introduction: The Medial Collateral Ligament (MCL) is a ligament located inside the knee joint that connects the femur to the tibia on the inner or medial side. The MCL's major role is to keep the knee joint stable and to avoid excessive sideways or valgus movements. Injury to the MCL might caused by valgus forces and sudden rotational extension of the knee, which frequently occurs during athletic activities causing the knee to be in a rotational position when the force is applied. As a result, the MCL is sprained or overstretched. If the force applied to the knee is greater, the ligament will rupture. Case Report: The patient is a 30-year-old male who has a ruptured right MCL. He had intraoperative surgery 1 day after his injury for reconstruction. After two weeks, the patient returned to the physiotherapy clinic, where he had difficulties bending and straightening his knee, the quadriceps’ muscle strength appeared to decrease, and he had difficulty in walking. Rehabilitation had been done twice a week and included activities to facilitate the improvement of joint range of motion and muscle strength. Conclusion: The present case concluded that the training program provided can enhance knee joint range of motion following MCL surgery. The findings of this case pointed to the necessity for bigger patient groups to be studied with long-term examination of the safety and efficacy for the MCL’s exercise program.
2023-09-22T00:00:00+07:00 Copyright (c) 2023 https://proceedings.ums.ac.id/index.php/apc/article/view/3138 Physiotherapy Management of Down Syndrome: Case Report 2023-10-14T13:49:02+07:00 Alfina Ramadani Dyah Efendi alfina.ramadani.ar@gmail.com Adnan Faris Naufal alfina.ramadani.ar@gmail.com Christine Viola alfina.ramadani.ar@gmail.com
Introduction : Down syndrome is a genetic condition or disorder caused by a chromosomal abnormality that occurs threefold in total or part of chromosome 21. Decreased muscle strength and balance are one of the consequences that occur in the condition of down syndrome. In this study the aim was to determine the effect of providing physiotherapy management in cases of Down syndrome. The research method used in this study was a case study involving a 4-year-old girl diagnosed with Down syndrome. Subjects were given physiotherapy interventions for 3 weeks with a frequency of 2 meetings a week. The interventions provided were walking on a textured catwalk, walking on an inclined plane, and jumping on a trampoline based on a stretch-shortening exercise. The examination given for this case is checking muscle strength using the manual muscle testing method and checking balance using the balance beam test. In this study, the results showed that there were changes in the subject's muscle strength and dynamic balance ability.
2023-09-22T00:00:00+07:00 Copyright (c) 2023 https://proceedings.ums.ac.id/index.php/apc/article/view/3139 Physiotherapy Management of Post Operating Achilles Tendon Ruptural (Phase III): A Case Study 2023-10-16T10:33:21+07:00 Siti Khadijah sk990@ums.ac.id Hakny Maulana sk990@ums.ac.id Nur Afni Agustyaningsih sk990@ums.ac.id Dika Tiara Salsabila sk990@ums.ac.id Nur Annisa sk990@ums.ac.id
Introduction: Physical injuries can disrupt the musculoskeletal system which includes muscles, bones, joints, tendons, ligaments, and the connective tissue that supports and binds tissues and organs together. The incidence of achilles tendon rupture has increased due to increased physical activity and sports, especially at the age of 30 and 40 years. Physiotherapy is a form of health service aimed at individuals and/or groups to develop, maintain and restore body movement and function throughout the life span by using manual handling, increased movement, equipment (physics, electrotherapeutic and mechanical), function training, and communication. Case Presentation: The patient was diagnosed with Achilles tendon rupture. The course of the disease was that the patient was playing mini soccer, when the patient ran sprints then his left foot stepped on it and a clicking sound was heard, after the injury the patient was still forced to walk, starting from there the patient felt quite disturbing pain, followed by edema in the left ankle and loss of joint stability. The physiotherapist performed a static inspection with the result that there was an incision along the left Achilles tendon, the patient's postural condition was good, there was visible edema of the left lateral malleolus. The results of a dynamic inspection check are that it can be seen that the running time is still based on the right side of the axle. Management and Outcome: The physiotherapy modality used is to provide cold compresses for 5 minutes. Before being given an exercise program, do a warm-up first. Then proceed with a static cycle for 15 minutes, closed chain exercise, lunges (3 sets), squad (3 sets), calf rises (3 sets), balance exercise, the single-leg brige (3 sets). When finished proceed with cool down. The results of this study were a decrease in edema which was evaluated by a figure of eight, but there was no change in muscle strength which was evaluated by MMT and functional ability which was evaluated by the FADI scale. Discussion: A decrease in edema can occur due to the icing modality or it can also be called a cold compress. Cryotherapy can show a decrease in edema, because cryotherapy can cause cold stimulation to skin tissue which can cause local vasoconstriction causing a decrease in edema fluid and lymph fluid production which can release inflammatory mediators through decreasing the permeability of blood vessel walls resulting in decreased edema. Close Chain Kinetic exercises provide activation of antagonistic groups of several joint group. Examples of closed chain exercises include pushups, pullups, squats, and lunges. All types of closed chain exercises can be performed whether using weights or not. Kinetic Closed Chain training focuses on eccentric muscle work, as it is known that eccentric training can develop higher muscle contractions in the muscles so that a greater training effect can be obtained and followed by increased functional performance. Conclusion: Management of physiotherapy programs given for 2 times resulted in the results that the initial therapy to the final therapy had not shown significant results. So it can be concluded that the left ankle edema has decreased, there is still muscle weakness, and there is no change in the patient's functional ability.
2023-09-22T00:00:00+07:00 Copyright (c) 2023 https://proceedings.ums.ac.id/index.php/apc/article/view/3140 The Effect of Physical Exercise on Muscle Strength in Chronic Rental Failure Patients during Hemodialysis: Article Review 2023-10-16T10:38:03+07:00 Fitria Rizky Anggraeni ardifaizin18@gmail.com Faizin Ardiansyah ardifaizin18@gmail.com Lintang Ayuninggar ardifaizin18@gmail.com Mandha Noor Fadjriyah ardifaizin18@gmail.com Afinda Krisdeana ardifaizin18@gmail.com Mahendra Wahyu Dewangga ardifaizin18@gmail.com
Introduction: Chronic kidney failure is a condition in which kidney tissue has progressively decreased function so that it is unable to maintain fluid balance in the body and requires therapy, namely hemodialysis. Improving physical work capacity and reducing functional limitations of muscle strength can be done through regular physical exercise in patients with chronic kidney failure after undergoing hemodialysis. Therefore it is necessary to conduct a systematic review study to determine the effect of physical exercise on chronic kidney failure patients. There are 5 articles used in this research. In carrying out the functions, the hip joint often experiences problems both as a result of trauma, degenerative, and post-traumatic hip. Osteoarthritis (90%) contributes the largest problems in hip joint, followed by femoral neck fracture (5%), avascular necrosis (2%), dysplasia (2%), and inflammatory arthritis (1%) (Ferguson et al, 2018). Thus, if conservative treatment did not provide better changes, then a total hip replacement is performed to reduce pain and improve physical function. Total hip replacement is a surgical procedure by replacing the entire hip joint with an implant. Implants themselves can last up to 20 years. Case Presentation: A 58-year-old patient with unable to endure pain and discomfort in his activities and at rest, when consulted by a doctor, patients experience infections around implants and plates & screws. After total hip replacement patient complained of pain in the incision on the side of tight and limited movement in the left leg. Management and Outcome: Early mobilization given in the form of breathing exercises, isometric exercises, and other exercises on the bed, then on the second and third days began to be given transfer-ambulation exercises. Pain is measured using NRS and joint scope of motion is measured using a goniometer. Discussion: From several articles that become supporting in line with the results obtained where the initial mobilization is a major influence in the patient's life in the future. Parents have an important role in stimulating child growth and development early on in child growth and development. Problems that occur in children of this age, one of which is gross motor development due to parents' lack of understanding of child development. Education in the form of health literacy discusses growth and development and how to stimulate according to age so that parents can apply what has been taught. The aim is to determine changes in the level of understanding of parents when given growth and development education about gross motor development of children aged 0-6 months. This study used a quantitative research design with a type of quay experimental research, a one- group pre-test-post-test approach. The number of samples was 44 respondents with the purposive sampling method. The variables of this study are independent, namely parents' understanding of child growth and development measured by questionnaires, and dependent variables, namely gross motor development of children aged 0-6 months measured by ASQ (Ages and stages questionnaire). Data analysis techniques used were univariate test, normality test, and effect test. The effect test using Wilcoxon showed the sig result (2-tailed) was 0.000 <0.05, meaning there is a significant effect.
